Comprehensive Crisis Intervention

Truwell's specialized team conducts thorough assessments and develops person-centered plans to address the root causes of crisis situations while providing essential relief.

Assess the person and situation to determine factors causing the crisis
Assist, supervise and provide care to ensure health and welfare
Develop person-centered intervention plans with the support team
Consult with and train providers and caregivers on implementation
Provide ongoing technical assistance for intervention plan execution
Recommend positive support strategies to prevent future crises
Develop transition plans for successful return home
Coordinate with healthcare providers and emergency services
Provide room and board in licensed foster care or CRS settings for out-of-home services
Ensure community stability and avoid institutional placement
Follow-up support and planning for long-term success

Crisis respite allows the person to avoid institutional placement
Individual can remain stable in the community with intervention
Enrolled in Minnesota waiver programs (CADI, DD, BI, CAC)
Experiencing behavioral or medical crisis requiring specialized intervention
